How to use this template
Before the meeting, fill in the ratings with at least one concrete example per competency. "Exceeds expectations" means nothing without evidence; "shipped the migration two weeks early" means everything.
Use the goals section to set two or three SMART goals for the next period — specific, measurable, with dates. Reviews that end with clear goals feel fair; reviews that end with vague feedback feel political.
Leave the employee comments section for them. A review is a conversation, and giving the employee the last written word builds trust in the process.
